The picture is kind of blurred so I cannot really say what caused it.

However with a wound like that infection is a serious threat.

Make sure your NH3 and NO2 is 0 and also low NO3 (below 20ppm is best)
Use medicine to prevent infection, I usually use API General Cure but anything with Metronidazole is good enough.
